In accordance with Article 4, Section 1340 of Title 15 any legal action, pertaining to conditions of confinement and filed against persons of legal entities responsible for juvenile facility operations shall be reported in writing to the Board of State and Community Corrections.

1. The Chief Probation Officer shall submit this report.  Copies of the report shall be distributed to:
   1. The Board of State and Community Corrections
   2. The Chief Administrative Officer
   3. The Juvenile Justice Commission
   4.  The Juvenile Court Judge
2. The report shall include:
   1. The nature of the allegations.
   2. The parties involved.
   3. The resolution or actions taken, if any.
