To request a demand from the San Luis Obispo County Department of Child Support Services, please submit your demand request by completing the Title Company Demand Request
All demand requests must include the following documents:
- A completed Title Company Demand Request form;
- A copy of the Abstract of Support Judgment (Lien) or Notice of Support Judgment; and
- A Release of Information Authorization signed by the Debtor.
A written demand will be provided within 15 days. Please note that verbal quotes cannot be provided.

Please make all checks payable to “California State Disbursement Unit" and include the participant's full name, participant number and “LIEN" on the check for proper identification and credit.
