Employer Paid for Select Bargaining Units Voluntary Supplemental Insurance Option | Pays beneficiaries a lump sum in the event of your death, and AD&D provides coverage if you suffer from a loss of a limb, speech, sight, or hearing, or if you die in an accident. |
Voya Long Term Disability Insurance Select Bargaining Units Only | Covers a percentage of your income if you cannot work due to an injury or illness preventing you from job functions over a long period of time. |
Aflac Voluntary Insurance Benefits
All Employees Eligible to Enroll | Adds protection for expenses related to an accident, including ER visits, physical therapy, and more. |
Aflac Critical Illness Insurance All Employees Eligible to Enroll | Pays a lump sum benefit upon initial diagnosis of a covered illness such as cancer, heart attack, or stroke. |
Aflac Hospital Indemnity Insurance All Employees Eligible to Enroll | Compensates a lump sum amount when confined to a hospital for planned or unplanned reasons. |

Monthly premiums vary depending on the type of insurance and amount of coverage selected. Monthly premium payments are deducted in two (2) installments from the first two (2) pay periods of each month. They appear as after tax deductions on the employee's remuneration statement.
