DOCUMENTS TO KEEP IN A CLOSED FILE

  • Narrative (Please update the narrative prior to closing the file)
  • Probation Questionnaire
  • Proof of Completion of any terms of probation, i.e., BIP, CWS, Residential Treatment, etc.
  • The most-recent Odyssey Case Summary
  • All minute orders from prior Court system
  • All DA Complaints/Information/Indictments
  • All filed Court documents
  • All Court documents/reports generated by Probation
  • All PRCS documents
  • All PPRF forms
  • All Static-99R Forms
  • All Facts of Offense Sheets
  • All requests for polygraph examinations, as well as the results of said examinations
  • All periodic reports from sex offender counselors

ALL OTHER DOCUMENTS CAN BE SHREDDED.
