2.6 CVA Identification Cards & Lanyards

Subject 2.6 CVA Identification Cards & Lanyards
Purpose To provide guidelines for issuing CVA identification cards and lanyards to youth in CVA and managing the identification cards and lanyards.

Overview

Youth admitted into the CVA shall be provided with a CVA identification card attached to a CVA lanyard. A duplicate/spare identification card shall be stored in CVA Unit Locker 17

CVA identification cards shall include a photograph of the youth in a CVA polo shirt, the youth’s first & last name, date of birth, Probation Number (P Number), a personal bar code (the Monitor Event Number connected with the youth’s booking into CVA), an expiration date, and the youth’s sex, height, weight, hair color, and eye color

Specific Procedure

  1. Each youth admitted into the CVA shall be provided with a CVA identification card and CVA lanyard
    1. CVA identification cards are to be attached to a CVA lanyard
    2. Staff may issue a Juvenile Hall wristband for youth in CVA who have not yet received a CVA identification card
  2. Staff will provide youth with their CVA identification card and CVA lanyard when transitioning off the CVA unit
    1. Staff shall inspect CVA identification cards and CVA lanyards prior to providing them to youth
  3. Youth may wear their CVA lanyard around their neck with the CVA identification card draped across their chest or keep their CVA identification card and lanyard in their pocket
  4. Youth must comply with requests to present or turn-in their CVA identification card when requested by any probation staff, San Luis Obispo County Office of Education (SLOCOE) staff, County Behavioral Health staff, Juvenile Hall Nursing Staff, or Family Care Network, Inc. (FCNI) staff
  5. Youth must return their CVA identification card and CVA lanyard to probation staff upon returning to the CVA unit
    1. Staff shall inspect CVA identification cards and CVA lanyards when collecting them back from youth
    2. Staff shall store youth’s CVA identification card and CVA lanyard in the designated collection location
  6. Youth must immediately report lost or damaged CVA identification cards and/or CVA lanyards to probation staff
    1. Youth who have accidently lost or damaged their CVA identification card and/or CVA lanyard shall be issued their spare CVA identification card and/or a new CVA lanyard (both located in CVA Unit locker 17)
      1. Staff shall document the incident on an Informational Report and notify a CVA JSO III
  7. Youth must not tamper with, alter, or destroy their CVA identification card or CVA lanyard.
    1. Youth who have purposefully tampered with, altered, or destroyed their CVA identification card and/or CVA lanyard shall be issued a Juvenile Hall wristband in lieu of a replacement CVA identification card and/or CVA lanyard
      1. Staff shall document the incident on a Non-Room Confinement Incident Report and notify a CVA JSO III
      2. Staff shall reissue a CVA identification card and/or CVA lanyard to the youth within 30 days
