Program evaluations are to be completed by adult probationers who have been in or are enrolled in Thinking for Change (T4C), Moral Recognition Therapy (MRT), Batterer’s Intervention Program (BIP), Children’s Safety Assurance Program (CSAP), and Directions.  Probationers are to do program evaluations upon completion of the program and, additionally, if a probationer is terminated from a program for any reason prior to completion. The program evaluation must be completed before the enrollment in Monitor can be closed.

  1. When a probationer either successfully completes a program or is terminated prior to completion, the program provider will submit a report to Probation.
  1. The assigned clerk will then process and scan the completion/termination in Monitor and forward to the Deputy Probation Officer (DPO) assigned to the case.
  1. The assigned DPO will schedule an appointment at Casa Loma or Prado to meet with the probationer within a week of receiving a notice of completion or termination. The evaluation is located in the forms area of monitor under the name enrollment evaluation.
  1. The DPO will provide the probationer with a program evaluation survey to complete during the meeting. The probationer can complete the survey in the lobby and turn in it at the window. The DPO does not need to be present during completion of the survey.
  1. The DPO will return the completed program evaluation survey to the assigned clerk, who will scan/save the document and process in Monitor:
    1. Enrollment Tab - Completed: Result = “Success”       Status = “Complete” OR
    2. Enrollment Tab - Terminated: Result = “Failed”         Status = “Terminated”
    3. Journal Notes - Completed: Type = “Meeting”           Result = “EvalDone”
  1. If the program was successfully completed, the assigned clerk will remove the corresponding probation term from the terms/conditions in Monitor. If the probationer was terminated prior to completion, the probation term will remain, and they will be advised by their DPO how to reenroll.
  1. The assigned clerk will forward the processed program evaluation survey to the SDPO overseeing department programs.
  1. Reviews by the SDPO and the Program Manager will take place following each round of T4C and MRT, and bi-annually for CSAP, BIP, and Directions.
