Read and comply with the Construction Requirements for Permanent Food Facilities reference document.

Submit the following:

• A completed Plan Check Application for Permanent Food Facility form
• One (1) copy of plans drawn to scale and with the scale indicated
• Equipment specification sheets and other information documents that will ensure compliance with equipment/construction requirements
• Samples of flooring material may be required
  • Review the approved flooring materials list to ensure proposed flooring material can be approved for use
• A completed Proposed Menu Items form

The permit fee is due with the application. Please refer to the Environmental Health Permit Fee Schedule for more information.

Plans must be approved prior to beginning construction. The inspector reviewing the plan will contact you with any additional information or forms that may be required, revisions needed, or when your plan is approved.

It is unlawful to begin operation of a new or remodeled Permanent Food Facility without first receiving approval from an inspector in addition to a valid Health Permit to Operate.

The inspector will grant approval to begin operation upon completion of construction and opening inspections.
