Purpose To establish guidelines for completing the following PRCS forms appropriately: CR300, CR301, and MC025.

The CR300, CR301, and MC025 State forms are in Monitor under the documents tab as “CR300,” ”CR301,” and “MC025.”

Monitor will pre-populate most of the CR-300 form once a violation has been entered into Monitor. Sections which need to be filled out include the CDCR number at the top right of the form, the prison sentence and County of conviction (Court case number if out of County) under #3, the specific violation of terms under #5 (please specify the term violated under the CR1515 or specific Special Conditions).  If the person is in custody, mark the “In Custody” box and add the location of where they are in custody, which would normally be “San Luis Obispo County Jail.”

For the MC025, only the Risk Assessment is pre-populated. The level of the risk needs to be in bold. The Summary of the offender’s history and progress, the Prior Record (you can copy and paste from previous PSI or PV Sentencing Report from the DPO’s F drive directory), Previous Violations, and your Recommendation need to be completed. This form needs to be in Ariel font size 10. This form needs to be electronically signed by the officer.

If you are writing a warrant, a CR301 Warrant Request form needs to be completed and electronically signed. The violation section needs to be brief and the entire completed form can be one page only. If an expanded violation summary needs to be written, this can be done on the MC025 form in the Summary section at the top of the form.

After you have completed the appropriate forms, please email your supervisor with the name of the offender and whether the report is for a revocation or a warrant. The supervisor will finalize and sign the MC025. Once the MC025 has been electronically signed by the supervisor, the documents will be sent electronically to the court clerks, DA, and public defender (assigned officer is cc’d).
