Vaccine Records 

You can get a Digital Vaccine Record online at the California Department of Public Health website, including vaccines you have received from our Public Health clinics.

Other Public Health Records 

If you would like to request records for other services you have received from the Public Health Department – such as reproductive health care, nurse home visiting, physical or occupational therapy, cancer screening, STD testing, TB testing, SART services, or other care – please contact us by email or call 805-781-5500.

Mental Health/ Drug & Alcohol Records

If you are looking for records of Mental Health or Drug & Alcohol treatment, please reach out to our colleagues at the Behavioral Health Department at 805-781-4724 or by email. If you are currently receiving Behavioral Health services, you may also speak with your provider to request records. 

Jail Medical Records 

You can request records of medical care you received while at the County jail. For care received prior to February 2019, please contact us by email or call 805-781-5500. 

For care received since February 1, 2019, you may complete this form and fax it to 805-781-5342 or mail it to Wellpath Medical Records, 1585 Kansas Avenue, San Luis Obispo, CA 93405.

General Hospital Records 

If you are seeking records from care you received at the County General Hospital that closed in 2003, you may reach out to your primary care provider from that time.
