What is the process?
Each User signs and returns User Agreement
Each user listed on the Web Portal User Spreadsheet must sign and return the Web Portal User Agreement to the County of SLO Public Health Laboratory by U.S. mail or courier (Dr. Glen Miller, SLO PH Laboratory, 2191 Johnson Ave., San Luis Obispo, CA 93401).
Supervisor Fills Out Web Portal User Registration
Supervisor must download, save and then fill out the Web Portal User Registration Spreadsheet (when prompted, select "SAVE" to download, save and print the spreadsheet) for each potential user. Once complete, email it to the County of San Luis Obispo Public Health Laboratory, [email protected], instructions are on the form.
Who is eligible?
Laboratories, facilities, physician's offices, clinics or water testing locations
Once your organization is registered and users have signed the user agreement, credentials will be emailed to users; refer to the Web Portal Instructions document for further information.
