Juvenile Hall Medical Release
Health Care services are provided to all youth in the Juvenile Hall. Licensed Health Care professionals, operating within the scope of their license and within facility health care services policy, have sole authority to make clinical decisions on individual youth under their care. In order to properly care for youth, Parent(s)/Guardian(s) need to sign a medical release form.

Parent(s) or Guardian(s) may obtain a Medical Release form online or in person at the Juvenile Hall. The completed form needs to be signed and returned to the San Luis Obispo Juvenile Hall.
Who is eligible?
The medical release form provided through this service is to be completed by the Parent(s)/Guardian(s) of the detained youth.
When and where is this service offered?
A parent or guardian may obtain a medical release form online or in person at the Juvenile Hall during regular business hours. The completed form needs to be signed and returned to the San Luis Obispo Juvenile Hall.
