Probation Statement of Loss - Juvenile

A Statement of Loss is a claim for which victims of crime who have suffered lost wages, crime-related losses, damaged or stolen property and have acquired medical expenses can request restitution.

Request Re-entry Into Extended Foster Care

Participation in foster care after the age of 18 is called Extended Foster Care.  Some youth, who were previously in foster care as wards of the Delinquency Court, are eligible to voluntarily return to foster care after the age of 18. 

 If you are an eligible youth who would like to request to re-enter Extended Foster Care, complete the attached JV-466 form. If you wish to keep information regarding your re-entry request confidential, complete the JV-468 form.  You can submit these forms through the Probation Department or directly to the Juvenile Delinquency Court.
