When a client is referred to the System Navigator Program, the assigned officer will complete the referral form and forward to Restorative Partners. Along with the referral form, the officer will provide Restorative Partners with the signed Release of Information (ROI) and the case plan created. On the ROI, the officer is to add Restorative Partners in the box marked other. The three documents (referral form, signed ROI, and case plan) will all be provided to Restorative Partners via email to Jonas ([email protected]) and Arthur ([email protected]). Arthur and/or Jonas will be sending in weekly updates on clients. Should the client’s behavior be of concern, a quicker method of contact will be utilized to the assigned officer.

Referral Form:

The officer will generate the System Navigator Referral Form through Monitor documents. The officer will ensure that each item on the list is discussed with the client and marked appropriately. All areas on form must be filled out. Once DMS saved, the document will then populate into the subfolder labeled “System Navigator” under “ASSMT/ REFERAL/ ENROLLMT”.


Enrollment:

In addition to creating the referral form, the assigned officer should open a new enrollment in Monitor. The enrollment must be opened on the day of the referral and updated after 60 days. Once the 60 days has passed, it is the officer’s responsiblility to document any updates.

Opening the Enrollment:
1. Click on “File”
2. Click on the “Enrollments” tab
3. Click on “New”. Drop down boxes will generate
4. Click on “Group” and choose “Referrals”
5. Click on “Type” and choose “System Navigator”
6. Click on “Agency” and choose “Restorative Partners”
7. Click on “To Start” and enter date of referral
8. Click on “To End” and enter a date of 60 days out
9. Click “Result” and choose “Pending”
10. Click “Save”
11. After 60 days, update as needed
