Handbooks are intended to provide general program information, explain your benefits, health resources, and answers to frequently asked questions.
Intake Forms and Packets
You will need to complete these forms when you are ready to start Behavioral Health services. Please note you may need to complete specific forms based on whether you are receiving mental health or substance use disorder treatment. Please check with Behavioral Health Counselors and Treatment staff if you have questions or concerns about intake forms.

Behavioral Health Provider Directories- SLO County
Below are directories which list names, locations, and phone numbers of Behavioral Health clinics and local providers. Provider Directories explain the services we offer and will help you find the resources you need. It is updated regularly for accuracy. Visit the Network Provider webpage to learn more about private practice services contracted with Behavioral Health and the referral process.

- All Behavioral Health clinic locations comply with Americans with Disabilities Act (ADA) requirements and are wheelchair accessible.
- All clinic locations accepting new clients.
- All clinics provide services in English and Spanish; other spoken language needs can be met as needed by contract with Language Line.
- American Sign Language are resources in brail are available as needed at all sites.
- Behavioral Health staff have specialized training to assist with a wide variety concerns and phases of life concerns (including early childhood, transitional aged youth, and older adult issues) that impact recovery. We use a trauma informed care model and can help with recovery from past abuse, trauma, or discrimination. All staff have received cultural competency training and are sensitive to culture, gender and sexual identity, and lifestyle choices and provide services in an open, caring, compassionate manner. Many staff are bilingual and/or bicultural.

Interpretation Services
The County of San Luis Obispo provides oral interpretation services from a qualified interpreter, on a 24- hour basis, at no cost to you. You do not have to use a family member or friend as an interpreter. We discourage the use of minors as interpreters unless it is an emergency. Interpreter, linguistic, and cultural services are available at no cost to you. Help is available 24 hours a day, 7 days a week. Special Aids
Free aids and services are available for to people with disabilities to help them communicate better. If you vision impaired, hearing impaired, having trouble reading please call TYY/TDD 1-800-735-2922. Upon request, documents can be made available to you in braille, large print, audio, CD, or other accessible electronic formats.
Patient's Right's Advocate
Patients' Rights Advocate helps Medi-Cal beneficiaries and other consumers understand their rights and access problem solving processes. Duties include:
- Providing written informational materials
- Advocating for consumers
- Investigating complaints
- Monitoring programs to be sure that patients' rights are protected
- Training providers and consumers about rights
- Communicating with the State Patients' Rights Program
Grievances and Appeals
If you believe that San Luis Obispo Behavioral Health Department has failed to provide these services or unlawfully discriminated in another way on the basis of sex, race, color, religion, ancestry, national origin, ethnic group identification, age, mental disability, physical disability, medical condition, genetic information, marital status, gender, gender identity, or sexual orientation, you can file a grievance by following the steps outlined on the Patients' Rights Advocate webpage.

FAQs
Some County of San Luis Obispo Behavioral Health programs may offer Remote Client Services. Remote Client Services enable you to continue program with a certified counselor via a virtual meeting platform called Zoom. Some Behavioral Health programs may require additional forms to be completed. Please reach out to your treatment team with questions and concerns.
In order to help you participate in Zoom Remote Client Services, we have created user-friendly instructions and tutorials found here. Your treatment team will be in contact with you over email with a web link so you may join your sessions virtually through a computer, tablet, or phone.
