Purpose To provide guidelines for when and how to develop a CVA Case Plan for youth in CVA.

Overview

An initial CVA Case Plan shall be finalized for every youth entering CVA within the first 45 days of entering the program. All case plans shall be developed with input from the youth’s CFT. Case plans will be reviewed to evaluate the youth’s progress toward meeting the objectives identified. Case plans expire within six months and must be replaced with a new case plan prior to expiration.

Specific Procedure

  1. Every youth ordered into CVA shall have a valid CVA Case Plan
    1. An initial CVA Case Plan shall be developed for each youth ordered into the CVA program
      1. Once a youth is ordered into CVA, the assigned CVA Deputy Probation Officer (DPO) shall convene a CFT meeting within the youth’s first 30 days in the program
      2. The initial CVA Case Plan shall be finalized within 45 days of the youth being ordered into the program
  2. Case plans shall be developed with input from the youth’s CFT members
    1. CFT members shall include the youth, the youth’s parent/guardian (optional if youth is 18), the assigned DPO, and the youth’s therapist
    2. CFT members may include others such as extended family, community and/or tribe, an educational representative, and a case manager
  3. Case plan objectives must address the youth’s Aftercare Plan, Permanency, and Safety Planning and shall incorporate objectives based on the youth’s highest scoring criminogenic risk factors identified in the youth’s most recent YLS / CMI Profile Report
    1. 1 – 3 risk factors for low and medium risk cases
    2. 3 – 4 risk factors for high risk cases
  4. Case plans shall be developed with Specific Measureable Achievable Realistic Timely (SMART) objectives and identify evidence based services to be provided
  5. Case plans are not valid until signed by a Supervising Deputy Probation Officer (SDPO)
  6. Case plans will be reviewed:
    1. To evaluate the youth’s progress toward meeting the objectives identified
    2. At 30 day intervals for youth in the in-custody portion of the program
      1. In-custody reviews will take place during weekly CVA Case Plan Meetings
        1. CVA Case Plan Meeting attendees shall include the youth, the youth’s assigned DPO, a SDPO, and the youth’s therapist
        2. CVA Case Plan Meeting attendees may also include an educational representative, a case manager, and medical staff
    3. At the youth’s CFT Meetings for youth in the in-custody portion or in the Aftercare portion of the CVA program
  7. Adjustments to the case plan shall be documented on the case plan and signed by the youth in the corresponding area of focus
  8. Case plans expire six months after being signed by a SDPO and must be replaced with a valid case plan prior to the expiration of the current case plan
    1. New case plans shall be developed in accordance with this procedure
